Disposal of Equipment
This page provides guidance for the removal of stolen, lost, or broken categorical equipment with a cost of $500 or more. Please select one of the 4 guides below and follow the instructions to dispose of equipment.
1- Salvaged Equipment (Updated Process)
  • Label your equipment as salvaged on the CEI (Section 3) by using the "Date of Disposal" and "Reason for Disposal" columns.
  • Computing Devices: Login to Service-Now and submit a ticket for "Technology Salvage/Recycling". Navigation once you login is: Employees > Submit a Request > Asset Management > Technology Salvage/Recycling.
  • Non-Computing: Transfer the items to the "Surplus Property & Recycling Annex" in Remedy and complete a Truck Operations request to have non-computing equipment picked up from their school.
2- Stolen Equipment (Updated Process)
  • Contact School Police and check if this qualifies for a police report. If so, label your equipment as stolen in the CEI (Section 3) by using the columns: "Date of Disposal", "Reason for Disposal" and "Safeguard Number". Include the police report number under "Reason of Disposal".
  • Report the item as Stolen in the Destiny system. Use the ITAM Guide for Stolen Items to guide you through the process. If you have any questions, comments or concerns, please contact ITAM.
  • If you have a Police report number, add it in Section 3 of the CEI under "Reason for Disposal". If School Police did not provide a police report number, then you can also use an iStar report number or a Remedy REQ/INC number.
3- Lost Equipment (Updated Process)
  • Contact School Police and check if this qualifies for a police report. If it qualifies, follow the "Stolen" instructions in the section above.
  • If the item is not stolen, report the item as Lost in the Destiny system. Use the ITAM Guide for Lost Items to guide you through the process. If you have any questions, comments or concerns, please contact ITAM.
  • If you have an iStar report number or a Remedy REQ/INC number, you should add it in Section 3 of the CEI under "Reason for Disposal". Otherwise, you should input "Reported as Lost in Destiny on MM/DD/YYYY".
4- Transfer Equipment (Updated Process)
  • ESSER Teacher Refresh Program (District Funded): Per the Teacher Refresh Project School FAQ, this device travels with teachers between schools and should be returned when they separate from the District. Pre-Authorization is granted to transfer this equipment by the COVID Fiscal Unit. Complete an ESSER Funded Device Refresh Project Transfer Form when this equipment transfers to a new school. Type "Teacher transferred to another school" under Disposition/Comments on your CEI. Keep the laptop on the CEI for one year before removing it. Receiving schools should add the items to the CEI. Submit the form [email protected] and "Cc" [email protected] in the same email.
  • ESSER School Funded Equipment: Contact the COVID Fiscal Unit office or FSEP for guidance and approval of transferring any school funded ESSER equipment.
  • Title I Funded Devices: Contact FSEP for guidance and approval before transferring any Title I Funded equipment.
  • ELOP Funded Equipment: Contact ELOP for guidance and approval before transferring any ELOP Funded equipment.
  • All equipment: Follow ITAM procedures for transferring equipment in Destiny.
Non-Technology
Equipment types that are not listed on Destiny. Some examples would be rugs, cabinets, musical instruments, science equipment, P.E. equipment and etc.
 
Use this form to report the items are lost or missing. This will provide you with a NT number that you can use.
